Payment Authorization Addendum

Terms

This Amendment to the iVET360 Service Agreement (“Amendment”) is made as of 09/09/2024 (the “Effective Date”) by and between iVET360, INC. of 222 NW 5th Avenue, Suite 4, Portland Oregon 97209 and my hospital.

WHEREAS, the parties desire to amend that agreement; and THEREFORE, in consideration of the foregoing and of the mutual covenants contains herein, my hospital and iVET360 hereby agree as follows:

1. Section 2 of the Agreement to amend and read as follows:

1.1 This Agreement is cancellable with 30-day notification. Notification by email will suffice. The user can email [email protected] at any time to cancel. Additional access beyond three free months can be extended at a rate of $100 per month. Payment authorization is required before the end of three free month trial to ensure uninterrupted access.

2. All other terms and conditions of the Agreement shall remain in full force and unaffected by this Amendment.

IN WITNESS WHEREOF, the Parties have hereto executed this Amendment to the iVET360 Services Agreement, effective as of the date set forth above.

Monthly Payment Authorization

My signature below authorizes iVET360, INC. to electronically debit my designated banking account or use the designated automatic credit card billing as the payment method for the services specified in the associated Services Agreement.

I authorize iVET360, INC. to electronically debit the below-designated banking account each month starting on the expiration of the three-month trial for $100 and recurring monthly until revoked.
